Application for Disbursement 2006-2007 Season Overview/Purpose The John & Thomas E. Burke Memorial Fund was
created at the request of former Melrose High School hockey star Tim Burke
in memory of his late brothers John and Thomas E. Burke. The fund is
supported through a generous donation from Tim to the Melrose Youth Hockey
Association. Applicant Requirements Any person applying for an assistance grant through the Burke Fund must meet the following requirements. Must be a Melrose resident Must be a member of the Melrose Youth Hockey Association Must fill out and submit a Burke Fund application to be considered for a disbursement award from the fund Must completely satisfy additional requirements for consideration as determined by the Melrose Youth Hockey Board of Directors Disbursement Types Disbursements from the John & Thomas E. Burke Memorial
Fund may take a variety of forms. Full Disbursement - a full disbursement would cover
all hockey related expenses (i.e., ice bill) for a player during a
specific season. Partial Disbursement - a partial disbursement would
cover some portion of the hockey related expenses for a player during a
specific season. Hardship Disbursement - a hardship disbursement would be a “one-time” disbursement brought about by an unexpected hardship or extraordinary circumstances. There will be no direct payments to recipients selected for disbursement from the fund. All disbursements will be applied to current balances owed to Melrose Youth Hockey by the individual or family. The Melrose Youth Hockey Board of Directors, when determining the type of disbursement for each application, will consider the following general criteria. It is important to note each application must be considered on a case-by-case basis due to the unique circumstances each situation may present. For this reason, the list below is more of a general guideline and should not be considered an all-inclusive list. Does the applicant meet all the Burke Fund
requirements? What is the nature and severity of the hardship
encountered by the applicant or applicant’s family? Can the severity and duration of the hardship be
verified with information provided through the fund application? Burke Fund Application Review Process The following process will be used when considering applications for disbursements or awards from the Burke Fund. The Burke Fund Application Review Committee
will review all applications. This committee will consist of the
President, Vice-President and Treasurer of Melrose Youth Hockey. The committee will fully review all submitted
applications, verify the information provided on the applications,
follow-up on any incomplete information and/or resolve any questions or
issues related to the application. Upon completion of the review process, the committee
will provide recommendations based on their review for the consideration
of the full Melrose Youth Hockey Board of Directors. The Board must
approve the committee’s recommendations related to an application in
order for the applicant to receive a disbursement or award from the
fund. An accounting of all disbursements from the fund will be sent, in
writing, to Mr. Tim Burke. All applicants will be notified in writing regarding
the final disposition of their request for assistance. The Melrose Youth Hockey Treasurer will be responsible for executing all disbursements of assistance funding from the Burke Fund.
